Prior to your first appointment please take the time to fill out the Health Care Form. If you are unable, please arrive 10-15 minutes prior to your scheduled massage to allow time to go over documents.
Please print and fill this form out prior to your first appointment. If you are unable to do so, please arrive 10-15 minutes early for your scheduled massage in order to fill out the form in the office.
